• We offer flexible financing options for our patients who choose to finance their laser vision correction or multifocal (bifocal) implants at the time of cataract surgery. Often, we can get loan approval in the office in minutes for our patients.

• Please bring your insurance cards at the time of service so that they may be photocopied.

• Office visit insurance co-payments are due at time of service.

• Self pay new patients are asked to pay for their office visit at the time of service. Otherwise, a minimum $60.00 deposit at time of service is expected. The remaining balance will be billed.

• Self pay established patients are asked to pay for their office visit at the time of service. Otherwise, a minimum $40.00 deposit at time of service is expected. The remaining balance will be billed.

• Refractions (glasses prescription exam) are considered routine eye care and are frequently not covered by insurance. Therefore, the fee for this service is the patient's responsibility.

• Contact lens related services are not covered by medical insurance. They are billed separately and may include a contact lens fitting fee, examination fee, and cost of replacement lenses.
